Pharmacist | Van Nuys, California

This opportunity is for a Pharmacist position specializing in Transitions of Care/Ambulatory Care at a 400-bed hospital in the San Fernando Valley region. The pharmacist in this role will play a key part in patient care by focusing on medication education and counseling prior to discharge. This approach supports patient adherence to medication regimens and promotes healthier outcomes after leaving the hospital, often referred to as a “meds to beds” program.

Key Responsibilities:

• Collaborate with patients before discharge to provide personalized medication education and counseling.
• Assist patients in obtaining medication refills to ensure continuity of therapy at discharge.
• Facilitate communication between patients and healthcare providers to support medication adherence.
• Support the hospital’s Transitions of Care initiatives to improve patient outcomes and reduce readmissions.

Desired Qualifications and Experience:

• Licensed Pharmacist with strong clinical knowledge.
• Preference given to new PGY1 residents, although non-PGY1 candidates will be considered.
• Experience or interest in ambulatory care and transitions of care programs is a plus.
• Ability to thrive in a busy hospital setting within a large public health system.

Additional Information:

• This role is within the LA County Department of Health System, which encompasses numerous outpatient clinics and hospitals.
• Pharmacists will be hired on a long-term contract basis through an agency; candidates will be on agency payroll but have the option to apply directly to work for the County in the future.
