Located in Van Nuys, CA, this contract opportunity offers a Pharmacist role focused on Transitions of Care/Ambulatory Care within a 400-bed hospital in the San Fernando Valley region. The position supports a large health system with multiple outpatient clinics and hospitals operated by the Los Angeles County Department of Health.

Responsibilities:

  • Provide medication education and counseling to patients prior to discharge.
  • Assist patients in refilling medications before leaving the hospital, supporting the 'meds to beds' program.
  • Enhance patient compliance with medication regimens after discharge to promote better health outcomes.
  • Collaborate within an interdisciplinary care team to ensure smooth transitions and continuity of care.

Desired Experience:

  • Pharmacists with recent completion of PGY1 residency preferred, though non-PGY1 trained candidates will be considered.
  • Strong communication skills for patient counseling and education.
  • Ability to work in an ambulatory care setting within a hospital system.
  • Commitment to improving patient outcomes during transitions of care.

Location: Van Nuys, California

Contract Length: Long-term contract assignment
