Travel Pharmacist - Waipahu, HI

A travel pharmacist opportunity is available in Waipahu, HI, serving the Honolulu area for a 16-week assignment starting ASAP. This role offers a dynamic work schedule with guaranteed 40 hours per week, rotating between day and evening shifts to provide variety and work-life balance.

Schedule:

• Day Shift Rotation: 5 days per week, 8-hour shifts (0630-1500 or 0830-1700)
• Evening Shift Rotation: 4 days per week, 10-hour shifts (1100-2130 or 1230-2300)

Key Responsibilities:

• Collaborate with medical teams during patient rounds to optimize drug therapy.
• Monitor patient responses and adjust medications accordingly.
• Provide pharmacokinetic dosing and patient counseling.
• Manage unit dose and intravenous (IV) services, ensuring accuracy and safety.

Desired Qualifications:

• Minimum of 2 years experience as a Clinical Hospital Pharmacist.
• Experience in medical team rounding, patient drug therapy monitoring, pharmacokinetics, and counseling.
• Familiarity with unit dose and IV services preferred.
• Active Hawaii State Pharmacist license.
• Doctor of Pharmacy (PharmD) degree preferred.
• Completion of a post-graduate clinical residency accredited by the American Society of Health-System Pharmacists is advantageous.
