A clinical pharmacist specialized in infectious diseases is sought for a direct hire opportunity in Warren, MI. This key role focuses on optimizing antimicrobial therapy and stewardship within a healthcare setting, ensuring patients receive safe, effective drug treatment.
Responsibilities include providing primary and consultative pharmacy services within the infectious diseases specialty, reviewing medication orders for accuracy and cost-effectiveness, and performing pharmacokinetic analyses with proper dosing adjustments. The pharmacist will actively participate in patient care rounds, collaborate with healthcare providers, and make evidence-based medication recommendations. Oversight and coordination of pharmacy staff, interns, and residents, as well as involvement in education, research, and committee activities, are integral to the position.
Key Duties:
- Lead and support Antimicrobial Stewardship Programs (ASP)
- Interpret microbiology data, cultures, and sensitivities
- Manage therapeutic drug monitoring (vancomycin, aminoglycosides, antifungals)
- Develop clinical guidelines, order sets, and formulary restrictions
- Educate clinicians on antimicrobial best practices
- Participate in medication use evaluations (MUE) and adverse drug reaction (ADR) activities
Qualifications:
- Doctor of Pharmacy degree required; PGY1 residency typically required
- PGY2 Infectious Diseases residency and/or Board Certification in Infectious Diseases Pharmacy (BCIDP) strongly preferred
- Licensure as a Registered Pharmacist and Controlled Substance registration in Michigan within 3 months of hire
- One to two years of clinical pharmacy practice in a healthcare environment preferred
- Strong background in microbiology, pharmacokinetics, and antimicrobial stewardship principles
