A pharmacy professional opportunity is available for a travel pharmacist at a 50-bed hospital in Northern Wisconsin. This position requires a dedicated pharmacist to support a healthcare team with clinical expertise and medication management on a rotating schedule.

Location: Northern Wisconsin (Washburn area)
Type: Contract, 13 weeks+
Schedule:

• 12-hour day shifts
• 7 days on, 7 days off rotation
• ASAP start

Responsibilities:

• Review medication orders to assess drug therapy, dose accuracy, dosage form, regimen, therapeutic duplication, and identify potential drug interactions
• Provide specialized pharmacokinetic and anticoagulation dosing guidance
• Participate in clinical pharmaceutical care including medication reconciliation and discharge medication education
• Oversee pharmaceutical compounding practices with compliance to USP regulatory guidelines, specifically USP 797 and USP 800
• Utilize EPIC electronic health records system for documentation and medication management

Qualifications/Desired Experience:

• Active pharmacist licensure appropriate for practice in Wisconsin
• Strong knowledge of clinical pharmacy practices, including medication therapy evaluation and patient education
• Experience with pharmacokinetic dosing and anticoagulation protocols
• Familiarity with USP standards for sterile and hazardous drug compounding (USP 797 and USP 800)
• Proficiency with EPIC or similar electronic medical record systems
• Ability to work independently and collaboratively within multi-disciplinary teams
