A California-licensed Pharmacist is needed for a remote Medication Therapy Management (MTM) pilot program focused on clinical pharmacy services. This part-time contract role offers flexibility with 10-20 hours per week, requiring residency in California and current enrollment in Medi-Cal as an Ordering, Referring, and Prescribing (ORP) provider. The position supports patient counseling and MTM efforts while helping to launch and maintain an innovative clinical pharmacy initiative.
Location: Remote (within California)
Schedule:
- Flexible hours, approximately 10-20 hours per week
- Day shift hours required for patient outreach and counseling
- Guaranteed minimum of 8 paid hours per week regardless of workload
Key Responsibilities:
- Conduct comprehensive MTM consultations with patients
- Provide medication education and patient counseling
- Review medication profiles and identify optimization opportunities
- Document clinical interventions and patient encounters meticulously
- Collaborate with program stakeholders to enhance patient outcomes
- Ensure compliance with all relevant state and federal regulations
Required Qualifications:
- Active and unrestricted California Pharmacist license
- Residency within California
- Previous experience in MTM services
- Current enrollment in California Medicaid (Medi-Cal) as an ORP provider
- Strong communication and patient counseling capabilities
- Ability to work independently and manage remote work responsibilities effectively
