A remote opportunity is available for a California-licensed Pharmacist participating in a Medication Therapy Management (MTM) pilot program designed to support clinical pharmacy initiatives across the state. This part-time contract role offers the flexibility to work 10-20 hours per week from any location within California, allowing you to balance professional contributions with your personal schedule.

Location:
• Remote (within California) — candidate must reside in California

Schedule:
• Flexible hours, approximately 10-20 hours weekly
• Day shift availability required to conduct patient outreach and counseling
• Guaranteed minimum of 8 paid hours per week regardless of workload

Responsibilities:
• Conduct MTM consultations to enhance patient medication use and outcomes
• Provide thorough patient counseling and medication education
• Review medication profiles to identify opportunities for therapy optimization
• Accurately document clinical interventions and patient interactions
• Collaborate with program stakeholders to improve patient care and support outcomes
• Ensure compliance with all applicable state and federal regulations

Qualifications:
• Active and unrestricted California Pharmacist license
• Residency in California is mandatory
• Previous experience with Medication Therapy Management (MTM)
• Enrollment as a California Medicaid (Medi-Cal) ORP provider (Ordering, Referring, and Prescribing) required
• Strong communication skills with the ability to effectively counsel patients
• Self-motivated and capable of working independently in a remote environment

This is a 3-6 month contract opportunity with a potential extension based on the pilot program’s duration and success. The role involves working within evolving workflows, providing the chance to shape clinical pharmacy services in a dynamic setting.
