Travel Pharmacist - Williston, VT

A travel pharmacist opportunity is available in Vermont with a community hospital affiliated with a major health network. This role supports a 100-bed facility with a focus on comprehensive pharmaceutical care. Candidates with strong IV experience and at least five years of pharmacy practice are encouraged to apply. Vermont offers a provisional license for out-of-state pharmacists, allowing candidates to begin work quickly and pursue permanent licensure during assignment.

Location:

• Berlin, Vermont
• 100-bed community hospital within a university-affiliated health network

Schedule:

• Monday through Friday
• Shift options include 6:30 AM to 3:00 PM, 10:00 AM to 6:30 PM, or 12:00 PM to 8:00 PM

Responsibilities:

• Perform medication reconciliation accurately and efficiently
• Verify medication orders for inpatient care
• Manage vancomycin dosing protocols and monitor related laboratory tests
• Adjust medications based on renal function and other patient parameters
• Apply strong IV pharmacy skills in daily tasks

Qualifications:

• Minimum of 5 years of pharmacy experience
• Proficiency in intravenous medication management
• Current Vermont pharmacist license or eligibility to obtain a provisional license immediately, with commitment to securing permanent licensure
• Demonstrated clinical skills with dosing adjustments, especially for antibiotics and renal dosing
