Pharmacist in Zachary, LA

A seasoned pharmacist is sought for a Medication Therapy Management (MTM) role within a specialty environment. This opportunity offers a 13-week contract with the potential for direct hire. The position provides flexibility to work onsite or remotely, requiring an active Louisiana pharmacist license alongside at least five additional licenses from a specified list of states.

Key Responsibilities:

• Prepare and dispense medication orders based on physician requests, adhering strictly to established protocols.
• Accurately interpret verbal and written prescriptions and update computerized patient medication profiles.
• Maintain thorough, precise pharmacy patient medication profiles.
• Safely dispense pharmaceuticals, including controlled substances, while maintaining mandatory records.
• Ensure safe, appropriate, and cost-effective drug therapies, monitoring for contraindications, interactions, allergies, and dosage issues.
• Detect and promptly report adverse drug reactions.
• Offer clinical consultations to healthcare practitioners, recommending therapeutic alternatives as needed.
• Document clinical interventions and activities completely and accurately.
• Participate in quality improvement and medication use reviews, including data collection and conducting inspections.
• Supervise pharmacy support staff, verify technician activities, and assist in performance appraisals.
• Organize and prioritize workflow to provide timely pharmacy services independently.
• Conduct medication storage inspections, ensuring removal of expired or unusable drugs.
• Professionally respond to telephone and in-person inquiries.

Qualifications:

• Current pharmacist license in Louisiana.
• Minimum of five additional active pharmacist licenses from the following states: Alaska, Arkansas, California, District of Columbia, Delaware, Hawaii, Indiana, Maine, Maryland, Massachusetts, Montana, New Hampshire, North Dakota, Oklahoma, South Dakota, Utah, Vermont, Virginia, West Virginia, Wyoming.
• Prior retail or hospital pharmacy experience.
• Commitment to obtain a total of 10 out-of-state licenses (beyond Louisiana) within 18 months in this role.
