Pharmacist in Zachary, LA

This opportunity is for a Pharmacist experienced in Medication Therapy Management within a specialty MTM environment. The role is contract-to-hire for approximately 13 weeks with the possibility of direct hire. The position offers flexibility with onsite or remote work options, located in Zachary, LA.

Key Responsibilities:

• Prepare and dispense medications according to physician orders and established protocols.
• Interpret verbal and written prescriptions, ensuring accurate entry into patient medication profiles.
• Maintain complete and accurate pharmacy patient records.
• Dispense pharmaceuticals and issue controlled substances while adhering to legal requirements.
• Monitor drug regimens for contraindications, interactions, allergies, and dosing accuracy.
• Detect and report adverse drug reactions promptly.
• Provide clinical consultation to healthcare practitioners and recommend cost-effective therapeutic alternatives.
• Document all clinical activities and contribute to quality improvement initiatives.
• Supervise pharmacy support personnel and verify daily pharmacy technician activities.
• Organize and prioritize work independently to ensure timely pharmacy services.
• Conduct medication storage inspections, removing outdated or unusable drugs.
• Maintain professional communication with patients and healthcare providers.

Qualifications & Desired Experience:

• Current Louisiana Pharmacist license is required.
• Must hold minimum of five additional pharmacist licenses from the following states or regions: Alaska, Arkansas, California, District of Columbia, Delaware, Hawaii, Indiana, Maine, Maryland, Massachusetts, Montana, New Hampshire, North Dakota, Oklahoma, South Dakota, Utah, Vermont, Virginia, West Virginia, Wyoming.
• Prior retail or hospital pharmacy experience.
• Ability to obtain a total of 10 state licenses (excluding Louisiana) within 18 months is expected.
