An established specialty and home infusion pharmacy is seeking 3-5 Patient Care Coordinators to join their on-site team in New Castle, DE. This contract-to-hire role offers an opportunity to play a vital part in patient care by acting as a liaison between the pharmacy, patients, providers, and caregivers. The ideal candidate will have solid pharmacy technician experience and thrive in a fast-paced, collaborative environment.

Location: On-site in New Castle, DE
Schedule:

  • 8:30 a.m. to 4:30 p.m.
  • and/or 9:00 a.m. to 5:00 p.m.

Contract duration: 13 weeks with potential for permanent hire based on performance

Key Responsibilities:

  • Facilitate upcoming medication orders by working closely with pharmacy staff, patients, healthcare providers, and caregivers
  • Conduct patient assessments during order refills, demonstrating clinical knowledge and attention to detail
  • Communicate effectively to ensure smooth workflow and accurate information exchange
  • Process insurance verifications and prior authorizations to support medication access
  • Utilize pharmacy software; experience with CareTend is preferred but not required

Qualifications and Desired Experience:

  • Minimum 1-2 years pharmacy technician experience, preferably in retail or long-term care settings
  • Strong understanding of pharmacy processes including dosage calculations and clinical basics
  • Knowledge of insurance procedures and prior authorization requirements
  • High School Diploma or GED required; national pharmacy technician certification highly preferred
  • Self-motivated and dependable with excellent communication skills
  • Able to work independently and as part of a focused team
