A home infusion specialty pharmacy is seeking 3-5 Patient Care Coordinators (PCCs) to join their on-site team in New Castle, DE. This contract-to-hire opportunity starts ASAP and runs for 13 weeks, with shifts from 8:30 AM to 4:30 PM and/or 9:00 AM to 5:00 PM.
This role serves as a vital liaison between the Pharmacy Department, patients, healthcare providers, and caregivers, ensuring timely facilitation of upcoming orders. Strong communication skills, attention to detail, and the ability to maintain an organized workflow are essential. Candidates will need a solid understanding of pharmacy processes, including dosage calculations and clinical basics to confidently interact with patients and complete necessary assessments with each order refill. Knowledge of insurance procedures and prior authorizations is also required.
Preferred candidates will have experience with CareTend software, though this is not mandatory. The ideal PCC is self-motivated, able to work independently, and also thrives within a collaborative team environment.
Responsibilities:
- Coordinate communication between pharmacy, patients, healthcare providers, and caregivers
- Manage and facilitate upcoming medication orders efficiently
- Perform required patient assessments during refill processes
- Apply knowledge of dosage calculations and clinical procedures
- Handle insurance verification and prior authorization processes
Qualifications:
- 1-2 years pharmacy technician experience, preferably in retail or long-term care
- High School Diploma or GED required
- Pharmacy licensing is not required in DE; national certification is highly preferred
- Experience with CareTend software is a plus
- Strong communication skills and reliability
- Self-motivated with a team-oriented mindset
