A home infusion specialty pharmacy is seeking experienced Patient Care Coordinators (PCCs) to join their onsite team in Newark, DE. This contract-to-hire opportunity offers a chance to contribute to patient care by serving as the vital link between the pharmacy department, patients, providers, and caregivers. The ideal candidate will demonstrate strong communication skills, attention to detail, and the ability to manage a focused workflow in a fast-paced environment.

Location: Newark, DE
Schedule: 8:30 AM – 4:30 PM and/or 9:00 AM – 5:00 PM
Contract Length: 13 weeks, with potential for full-time hire based on performance
Start Date: Immediate

Responsibilities:

  • Serve as liaison between pharmacy staff and patients, healthcare providers, and caregivers to coordinate upcoming orders
  • Ensure smooth communication and timely follow-up on patient orders and assessments
  • Complete required patient assessments with each order refill, demonstrating knowledge of clinical basics
  • Calculate medication dosages accurately and verify pharmacy processes
  • Navigate insurance requirements, including prior authorizations to facilitate order processing
  • Maintain an organized workflow while meeting daily productivity metrics

Qualifications & Desired Experience:

  • Minimum 1-2 years pharmacy technician experience, preferably in retail or long-term care settings
  • High School Diploma or GED required
  • Pharmacy technician licensure is not required in DE, but national certification is highly preferred
  • Strong familiarity with insurance procedures and prior authorizations
  • Experience using CareTend software is a plus, but not mandatory
  • Excellent communication skills with the ability to interact compassionately and professionally with patients and healthcare professionals
  • Self-motivated with a commitment to reliability and a career-oriented mindset
  • Ability to thrive both independently and as part of a collaborative team
