Join a remote team dedicated to enhancing patient medication adherence as a Patient Outreach Specialist in a value-based pharmacy operation. This contract role offers an opportunity to make a meaningful impact by coordinating patient-related needs through telephonic outreach while working from the comfort of your home.
Location: Remote (Candidates must reside in Eastern or Central Time Zones)
Contract Duration: Approximately 3-4 months (starting July 27 through mid-November)
Schedule: Monday to Friday, 8:30 AM – 5:00 PM ET (flexibility for occasional afternoons, evenings, and Saturday mornings)
Key Responsibilities:
- Conduct inbound and outbound calls to patients, healthcare providers, and pharmacies to gather medical information and support medication adherence, focusing on conditions such as hypertension, cholesterol, and diabetes.
- Streamline pharmacy processes with an emphasis on continuous improvement.
- Foster strong communication and relationships among stakeholders to facilitate effective care coordination.
- Accurately document patient interactions and comprehensive post-call notes using electronic health record (EHR) systems.
Desired Qualifications and Experience:
- Minimum of 6 months experience as a pharmacy technician; call center or telephonic support background preferred.
- Strong verbal communication skills with exceptional phone presence — absolutely essential for patient engagement.
- Basic knowledge of Medicare and STAR ratings.
- Comfort with technology including EHR platforms, Microsoft Excel, and Google Suite tools.
- Excellent organizational skills, attention to detail, and ability to multitask effectively.
- Self-motivated with a proactive attitude, capable of thriving in a fast-paced remote environment.
- Reliable high-speed internet (minimum 100 Mbps download, 10 Mbps upload) with Ethernet connection (Wi-Fi not permitted).
Work from Home Requirements:
- Dedicated private workspace with minimal background noise, equipped with a desk and chair.
- Client will provide necessary workstation equipment, which must be returned at contract end.
