Join a specialized healthcare environment supporting patients through home infusion therapy as a Patient Care Coordinator in New Castle, DE. This role focuses on being the vital communication link between the pharmacy team, patients, providers, and caregivers to ensure timely and accurate order processing.

Key Responsibilities:

  • Facilitate order coordination between pharmacy, patients, providers, and caregivers
  • Perform patient assessments and verify clinical information with each order refill
  • Apply pharmacy knowledge including dosage calculations and clinical basics to support patient care
  • Navigate insurance processes and manage prior authorizations efficiently
  • Maintain organized workflows with a strong attention to detail
  • Communicate effectively to support successful patient outcomes

Qualifications and Experience:

  • Minimum 1-2 years of pharmacy technician experience; retail or long-term care backgrounds preferred
  • Demonstrated understanding of pharmacy processes and clinical fundamentals
  • Knowledge of insurance procedures and prior authorizations essential
  • Experience with CareTend software is a plus, but not mandatory
  • High School Diploma or GED required; national pharmacy technician certification highly preferred
  • Self-motivated with the ability to work independently and collaboratively within a focused team

Schedule:

  • Shift options: 8:30 AM–4:30 PM and/or 9:00 AM–5:00 PM

This is a contract-to-hire opportunity starting immediately and lasting approximately 13 weeks.
