This opportunity is for experienced Ambulatory Clinical Pharmacy Specialists who are eager to deliver advanced outpatient clinical services in a collaborative healthcare setting. Schedule:
- Monday through Friday
- 8:00 AM to 4:30 PM or 8:30 AM to 5:00 PM
- No weekends, no holidays
Key Responsibilities:
- Deliver ambulatory clinical pharmacy services including medication assessment, monitoring, and documentation under pharmacy Collaborative Practice Agreements
- Conduct patient visits focusing on medication reconciliation, lab monitoring, patient counseling, and disease state management
- Collaborate with multidisciplinary care teams to implement evidence-based medication interventions that enhance quality, safety, and cost-efficiency
- Manage referrals with timely communication back to healthcare providers
- Facilitate patient medication access through prior authorizations and financial assistance programs
- Oversee specialty medication therapy including initiation, monitoring, and continuity of care
- Provide drug information and education to patients and healthcare team members
- Precept pharmacy students, interns, and residents as assigned
- Participate in quality improvement, research, and policy development initiatives
Qualifications:
- Active Delaware Pharmacist License in good standing
- Board of Pharmacy Specialties (BPS) Certification in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics (mandatory)
- Minimum one year of clinical pharmacy experience including residency
- Bachelor’s degree in Pharmacy or Doctor of Pharmacy (PharmD strongly preferred)
- Experience with Collaborative Practice Agreements and specialty medication management preferred
- Residency completion (PGY1 or PGY2 ambulatory care) is advantageous
