Staff Pharmacist - Chandler, Arizona

A clinical pharmacist specializing in home infusion will play a critical role in managing and dispensing complex intravenous (IV) and subcutaneous medications for patients. This position involves preparing sterile compounded medications and acting as a key member of a multidisciplinary team dedicated to ensuring optimal patient outcomes and safety.

Location: Chandler, AZ
Job Type: Contract (8-12 weeks)
Schedule:

• Monday to Friday, 8:30 AM–5:00 PM
• On-call rotation may be required

Key Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions in a sterile environment compliant with all state and federal regulations.
• Monitor patient lab values and calculate pharmacokinetic dosing, including medications like vancomycin and Total Parenteral Nutrition (TPN).
• Provide detailed patient and caregiver education on infusion pumps, sterile techniques, and catheter maintenance.
• Collaborate with physicians, home health nurses, and pharmacy teams to coordinate care and reduce hospital readmissions.
• Ensure pharmacy compliance with regulatory standards such as State Board of Pharmacy, HIPAA, DEA, Medicare, ACHC, URAC, OSHA, and others.
• Manage inventory, including refrigerated medications, and prepare shipments for timely delivery to patient homes.

Qualifications and Desired Experience:

• 1 to 3 years of experience in infusion specialty pharmacy or hospital pharmacy with primary involvement in the pharmacy IV room.
• Experience with IVIg therapies is a plus.
• Understanding of specialty therapy payer requirements, reimbursement procedures, and contract compliance.
• Current and valid pharmacist license in Arizona and Texas, in good standing.
• Familiarity with the CareTend system preferred.
