This direct hire opportunity is for a skilled staff pharmacist to join a closed-door long-term care (LTC) pharmacy located in Mason, OH, a serene suburb northeast of Cincinnati. The role provides a dynamic mix of clinical, operational, and regulatory responsibilities designed to optimize patient outcomes and ensure compliance within a collaborative healthcare environment.
Key Responsibilities:
- Review, verify, and document new patient orders while working closely with physicians and healthcare professionals to plan and implement safe, effective drug therapies.
- Conduct thorough assessments of patient-specific drug and disease information to identify potential medication-related problems and support therapeutic goal setting.
- Monitor drug therapy outcomes by evaluating lab data and clinical indicators, communicating findings to healthcare teams.
- Provide expert consultation to patients, families, and medical staff regarding medication use.
- Perform operational duties including medication processing, record maintenance in line with legal standards, and participation in staff training and meetings.
- Dispense pharmaceuticals and supplies adhering to federal, state, and local laws and professional practice standards.
- Engage in weekend, holiday, and on-call rotations as part of the pharmacy team.
- Maintain up-to-date clinical knowledge through ongoing education and training.
Qualifications:
- 1-3 years of experience in a long-term care pharmacy setting.
- Familiarity with Frameworks software is highly preferred but not mandatory.
- Strong clinical assessment and communication skills.
- Ability to maintain meticulous documentation and comply with regulatory requirements.
