Per Diem Staff Pharmacist - Columbus, Ohio

A specialty retail pharmacy is seeking a Per Diem pharmacist to support their Columbus, OH location. This role focuses on providing expert medication management and consultation for patients with complex conditions, many of whom are on 8-15 daily medications. Strong communication, empathy, and in-depth knowledge of chronic disease state management are essential in delivering personalized care.

Schedule:

• Tuesday, 2/3, 8:30 AM – 5:00 PM
• Thursday, 2/5, 8:30 AM – 5:00 PM
• Tuesday, 2/10, 8:30 AM – 5:00 PM
• Thursday, 2/12, 8:30 AM – 5:00 PM
• Friday, 2/13, 8:30 AM – 5:00 PM

Key Responsibilities:

• Provide thorough medication reviews and patient consultations for complex cases.
• Manage prescriptions while paying close attention to multiple drug therapies.
• Support patients with chronic and advanced diseases such as diabetes, hypertension, and multiple sclerosis.
• Deliver compassionate care tailored to Medicare/Medicaid and low-literacy populations.
• Utilize Pioneer Rx software efficiently to ensure accuracy and compliance.

Desired Experience and Skills:

• Strong background in retail and/or specialty pharmacy settings.
• Proficiency in disease state management and patient counseling.
• Exceptional patience and communication skills to engage with low-literacy and vulnerable patients.
• Ability to understand and manage complex medication regimens.
• Prior experience working with chronic disease states and Medicare/Medicaid populations.
