A direct hire opportunity is available for two Ambulatory Clinical Pharmacy Specialists in a leading healthcare system in Dover, Delaware. This role offers a meaningful clinical position focused on outpatient pharmacy services with a schedule of Monday through Friday, no weekends or holidays, allowing for a balanced work-life rhythm.
Responsibilities include providing comprehensive ambulatory clinical pharmacy care as a credentialed member of the healthcare team under Collaborative Practice Agreements (CPAs). The specialist will conduct medication management visits, collaborate with providers on evidence-based medication interventions, and oversee specialty medication therapies. Additional duties encompass supporting patient access to medications through prior authorizations and financial assistance programs, precepting pharmacy students and residents, contributing to research and quality improvement initiatives, and documenting all interventions in the electronic health record.
Key Qualifications:
- Active Delaware Pharmacist License in good standing (or willingness to obtain promptly)
- Board of Pharmacy Specialties (BPS) certification in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics (mandatory)
- Minimum of one year clinical pharmacy experience, including residency
- Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD preferred)
- Experience with Collaborative Practice Agreements and ambulatory care pharmacy strongly preferred
- Specialty medication management and prior authorization experience are advantageous
- Preceptor experience is a plus
Schedule:
- Monday to Friday
- 8:00 AM to 4:30 PM or 8:30 AM to 5:00 PM
- No weekends or holidays
