A direct hire opportunity is available for a Staff Pharmacist at a closed-door long-term care (LTC) pharmacy located in Mason, OH, a northeast suburb of Cincinnati. This role offers an engaging blend of clinical and operational responsibilities within a supportive and patient-focused setting.
Responsibilities include conducting thorough reviews and verifications of all new patient orders, collaborating closely with physicians and healthcare professionals to optimize patient care in the home, and performing initial and ongoing patient assessments to identify drug-related problems. The pharmacist will also monitor drug therapy outcomes by evaluating laboratory and clinical data, communicate findings to the healthcare team, and provide expert consultation to patients and caregivers. Maintaining detailed documentation and staying current with pharmacotherapy knowledge through continuing education are essential components of the role.
Operational duties involve processing and checking medications and supplies prior to dispensing, maintaining accurate pharmacy records, participating in weekend, holiday, and on-call rotations, training new and existing staff, and attending department meetings. Regulatory responsibilities include dispensing pharmaceutical supplies and equipment in compliance with all relevant laws and standards.
Key Qualifications and Desired Experience:
- Minimum of 1-3 years’ experience in a long-term care pharmacy setting
- Strong clinical assessment abilities related to drug therapy management
- Familiarity with Frameworks system is highly preferred, but not mandatory
- Demonstrated commitment to maintaining accurate documentation and adhering to regulatory standards
- Effective communication skills for collaboration with healthcare teams and patient consultation
