Staff Pharmacist - Fairfield, OH

This direct hire opportunity is for a Staff Pharmacist position within a closed-door long-term care (LTC) pharmacy located in Mason, OH, a northeast suburb of Cincinnati. The role focuses primarily on clinical pharmacy services, supporting patient-centered care in home settings while ensuring safe and effective medication management.

Key Responsibilities:

• Review, verify, and document new patient medication orders in compliance with organizational policies.
• Collaborate with physicians and healthcare teams to plan, implement, and monitor therapeutic drug regimens.
• Conduct thorough assessments of patient-specific drug and disease information to identify potential medication-related issues.
• Assist in setting therapeutic goals and monitoring parameters for safe and efficacious drug therapy.
• Interpret clinical and laboratory data to monitor drug therapy outcomes and communicate findings to medical staff.
• Provide drug information and counseling to patients, families, and healthcare professionals.
• Maintain detailed documentation of all clinical activities and communications.
• Participate in the dispensing process by checking medications, supplies, and equipment before distribution.
• Ensure adherence to all regulatory requirements, including federal, state, and local laws.
• Participate in weekend, holiday, and on-call rotations, as well as staff training and department meetings.

Qualifications / Desired Experience:

• Minimum of 1-3 years of experience in a long-term care pharmacy setting.
• Familiarity with the Frameworks pharmacy system is highly preferred but not mandatory.
• Strong clinical skills with the ability to assess and manage complex patient medication regimens.
• Excellent communication and teamwork abilities.
• Commitment to maintaining up-to-date pharmacotherapy knowledge through ongoing education.
