Staff Pharmacist - Gilbert, AZ

A home infusion clinical pharmacist is needed to support preparation, dispensing, and monitoring of complex intravenous (IV) and subcutaneous medications. Working as a key member of a multidisciplinary care team, this role focuses on sterile compounding, clinical dose adjustment, and patient education to ensure safe and effective home infusion therapies.

Schedule:

• Monday to Friday, 8:30 a.m. to 5:00 p.m.
• May require participation in on-call rotation

Key Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions in a sterile environment adhering to all relevant regulations
• Assess lab values, calculate pharmacokinetic dosing (e.g., vancomycin), monitor Total Parenteral Nutrition (TPN), and recommend clinical interventions
• Educate patients and caregivers on infusion pump use, sterile techniques, and catheter maintenance
• Collaborate with physicians, home health nurses, and internal departments to coordinate therapy and prevent hospital readmissions
• Ensure compliance with regulatory bodies including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA
• Manage pharmacy inventory, including refrigerated products, and prepare medication shipments for safe delivery using the CareTend system

Qualifications and Experience:

• Current, valid pharmacist license in Arizona and Texas in good standing
• 1-3 years’ experience in infusion specialty pharmacy or hospital pharmacy with primary responsibility in pharmacy IV room
• IVIg experience is a plus
• Familiarity with specialty therapy payer requirements, reimbursement, and contract obligations
