A healthcare facility in Glenwood Springs, CO is seeking an experienced pharmacist to support medication management during their EMR transition. This contract position offers a 20-week assignment with a start date of March 17. The role involves working rotating shifts Monday through Friday, starting at either 6:30 AM or 8:00 AM, plus rotating weekends from 7:00 AM to 12:00 PM, totaling 40 hours per week.

Key Responsibilities:

  • Prepare and dispense medications based on physician orders, including IV admixtures, chemotherapy, and parenteral nutrition.
  • Maintain accurate computerized medication profiles and comply with all legal regulations concerning medication handling.
  • Monitor drug therapies for interactions, allergies, contraindications, and dosage appropriateness.
  • Perform pharmacokinetic drug dosing and review culture and sensitivity data to ensure antibiotic appropriateness.
  • Provide clinical consultation to medical staff and communicate drug information to patients and families.
  • Participate in quality improvement initiatives, medication review processes, and serve as a member of the Code Response Team.
  • Support educational efforts by training pharmacy, nursing, and medical personnel as needed.

Qualifications:

  • Active Colorado Pharmacist License in good standing.
  • Graduate from an ACPE-accredited School of Pharmacy with a Bachelor of Science in Pharmacy.
  • Current BLS and ACLS certifications from the American Heart Association.
  • Flu vaccination is mandatory (no exemptions) during flu season (Oct 1 – April 1).
  • Experience with EPIC electronic medical records system is required.
  • Previous exposure to Meditech EMR is a plus, as the facility is transitioning systems during this assignment.
