A direct hire opportunity is available for a Staff Pharmacist specializing in closed-door long-term care pharmacy services. This role is based in Mason, OH, a northeast suburb offering a dedicated healthcare environment focused on safe and effective medication management.

Responsibilities:

  • Review and verify all new patient medication orders, documenting accurately.
  • Collaborate with physicians, nursing staff, and other healthcare professionals to optimize patient drug therapy.
  • Conduct thorough assessments of patient drug regimens and disease states to identify potential drug-related problems.
  • Assist in establishing therapeutic goals and monitoring parameters for drug therapy.
  • Monitor and interpret laboratory and clinical data to evaluate drug therapy outcomes.
  • Provide drug-related consultation to healthcare teams, patients, and families.
  • Maintain documentation of all clinical activities and recommendations in patient records.
  • Keep abreast of current pharmacotherapy literature, in-services, and educational sessions.
  • Implement appropriate formulary substitutions and match drug therapy with diagnoses.
  • Manage medication processing, verification, and pharmacy recordkeeping.
  • Participate in weekend, holiday, and on-call rotations.
  • Engage in staff training, orientation, and department meetings.
  • Ensure compliance with federal, state, and local pharmacy laws and standards when dispensing medications.

Qualifications:

  • 1-3 years of pharmacy experience within a long-term care setting.
  • Familiarity with Frameworks pharmacy system is highly preferred, though not mandatory.
  • Strong clinical assessment and drug therapy monitoring skills.
  • Excellent communication and documentation abilities.

Location:

  • Mason, OH (Northeast suburb of Cincinnati).
