A direct hire opportunity is available for an experienced home infusion clinical pharmacist in the Memphis, TN area. This role requires a dedicated professional to prepare, dispense, and monitor complex intravenous (IV) and subcutaneous medications, playing a crucial role within a multidisciplinary healthcare team.
Location: Memphis, TN
Schedule: Monday to Friday, 8:00 AM to 5:00 PM
On Call: Participate in an on-call rotation with typically fewer than 5 after-hours calls per week
Key Responsibilities:
- Prepare sterile IV solutions and specialty pharmaceuticals in compliance with federal and state regulations
- Monitor clinical laboratory values and pharmacokinetic dosing, including for medications such as vancomycin and Total Parenteral Nutrition (TPN)
- Provide thorough patient counseling and education about infusion therapies and medication regimens
- Instruct patients and caregivers on proper use of infusion pumps, sterile techniques, and catheter care at home
- Coordinate with physicians, home health nurses, and internal teams to ensure optimal therapy management and reduce hospital readmissions
- Maintain compliance with regulatory standards including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA
- Manage pharmacy inventory, especially refrigerated products, and ensure timely delivery of medications to patients’ homes
Desired Qualifications and Experience:
- Minimum of 3 years’ infusion specialty pharmacy experience
- Highly preferred experience with IVIg and hemophilia therapies
- Familiarity with specialty therapy payer requirements, reimbursement processes, and contract management
- Current and good standing Tennessee pharmacist license
- Experience with CareTend system is advantageous
