A home infusion clinical pharmacist position is available for a direct hire opportunity with an immediate start. This role demands expertise in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications within a home infusion setting. The pharmacist will serve as a key member of a multidisciplinary team focused on providing exceptional patient care, ensuring medication safety, and fostering effective communication among healthcare providers and patients.
Schedule:
- Monday through Friday, 8:00 AM – 4:30 PM
- Participate in on-call rotation
Responsibilities:
- Prepare specialty pharmaceuticals and IV solutions in a sterile environment, adhering to state and federal guidelines.
- Monitor lab values, calculate pharmacokinetic dosing (such as vancomycin), and oversee Total Parenteral Nutrition (TPN) to optimize treatment plans.
- Educate patients and caregivers on infusion pump usage, sterile techniques, and catheter maintenance.
- Collaborate with prescribing physicians, home health nurses, and internal teams to ensure coordinated therapy management and reduce hospital readmissions.
- Maintain compliance with relevant regulations including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
- Manage pharmacy inventory, including refrigerated products, and oversee packaging and shipping for safe patient delivery.
Qualifications:
- Minimum of 2 years’ experience in infusion specialty pharmacy
- IVIg experience is a plus
- Comprehensive knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations
- Current and active NY pharmacist license in good standing
- Experience with CareTend system preferred
