Home Infusion Staff Pharmacist | Baldwinsville, New York

A Clinical/Infusion Pharmacist role is available supporting home infusion therapy within a multidisciplinary team. This direct hire opportunity provides a dynamic environment where you oversee the preparation, dispensing, and clinical monitoring of complex intravenous (IV) and subcutaneous medications for patients managing care at home.

Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions under sterile conditions, ensuring strict adherence to state and federal regulations.
• Monitor laboratory values and calculate pharmacokinetic dosing, including vancomycin, and manage Total Parenteral Nutrition (TPN) adjustments.
• Provide detailed counseling and education to patients and caregivers on medication regimens, infusion pump use, sterile techniques, and catheter maintenance.
• Collaborate with physicians, home health nurses, and internal teams to coordinate care and optimize patient outcomes while minimizing hospital readmissions.
• Ensure compliance with regulatory standards such as State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
• Manage pharmacy inventory including refrigerated items and coordinate shipping to patients’ homes.

Qualifications and Desired Experience:

• Minimum of 2 years’ experience in infusion specialty pharmacy.
• IV Immunoglobulin (IVIg) experience is a plus.
• Familiarity with specialty therapy payer requirements, reimbursement, and contract obligations.
• Current New York pharmacist license in good standing.
• Experience with CareTend system preferred.
• Ability to participate in an on-call rotation.

Schedule:

• Monday through Friday, 8:00 AM to 4:30 PM.

Location:

• This position is based in Baldwinsville, NY, supporting operations in Syracuse, NY.
