Home Infusion Staff Pharmacist IV Certified in Bronx, NY

A healthcare facility in the Bronx, NY, is seeking two experienced pharmacists for a 13-week contract with potential to convert to permanent positions based on performance. This role requires a pharmacist licensed in New York with IV certification and prior home infusion pharmacy experience. The shifts are Monday through Friday, 8:30 a.m. to 5 p.m., with immediate start.

Key Responsibilities:

• Accurately accept and dispense prescription medications while monitoring patient compliance and over-usage.
• Ensure that medication supplies comply with all legal requirements.
• Maintain confidential and up-to-date patient records and pharmacy documentation.
• Provide consultation on prescription and over-the-counter medications, including dosage, side effects, storage, and drug interactions.
• Educate patients regarding proper use of medical devices such as blood pressure monitors and glucose meters.
• Offer additional patient services such as smoking cessation and cholesterol management.
• Compound medications for internal and external use and prepare sterile solutions following established procedures.
• Supervise pharmacy staff including technicians and clerks.
• Coordinate with healthcare providers to address drug-related problems effectively.

Qualifications and Desired Experience:

• Doctor of Pharmacy (PharmD) degree.
• Active New York State pharmacy license.
• IV pharmacy certification and prior IV pharmacy experience.
• Experience in home infusion pharmacy preferred.
• Strong communication skills and attention to detail.

Schedule:

• Monday to Friday, 8:30 a.m. to 5:00 p.m.
