Home Infusion Staff Pharmacist - Brooklyn, New York

A healthcare facility in Brooklyn, NY is seeking two licensed pharmacists with IV certification for a 13-week contract, with the potential to transition to permanent roles based on performance. This position requires a strong background in home infusion pharmacy and is ideal for pharmacists ready to start immediately.

Location: Brooklyn, NY
Job Type: Contract (13 weeks, possible contract to hire)
Schedule: Monday to Friday, 8:30 AM – 5:00 PM

Key Responsibilities:

• Dispense prescription medications accurately and monitor patient medication compliance.
• Ensure all medicines supplied comply with legal standards and are appropriate for patient needs.
• Maintain confidential patient records and pharmacy documentation rigorously.
• Provide patient counseling on medication usage, potential reactions, side effects, and interactions.
• Deliver additional patient services such as smoking cessation support, blood pressure monitoring, and cholesterol management.
• Educate patients on using medical devices including blood pressure monitors and glucose meters.
• Compound medications for both internal and external use and prepare sterile solutions under proper protocols.
• Supervise pharmacy technicians and clerks to maintain a high standard of pharmacy operations.
• Monitor inventory of pharmaceutical and medical supplies to ensure availability.
• Collaborate effectively with physicians, nurses, and other healthcare professionals to resolve drug-related problems.

Qualifications and Experience:

• Doctor of Pharmacy (PharmD) degree required.
• Current New York State pharmacy license must be in-hand.
• Prior IV experience is mandatory.
• Experience in home infusion pharmacy preferred.
