A Staff Clinical/Infusion Pharmacist role is available for an 8-12 week contract focused on home infusion therapy. This position requires expertise in preparing and dispensing complex intravenous (IV) and subcutaneous medications while serving as a key clinical resource to patients, caregivers, and multidisciplinary teams.

Key Responsibilities:

  • Sterile Compounding: Prepare specialty pharmaceuticals and IV solutions in a sterile environment adhering to all regulatory standards.
  • Clinical Monitoring: Evaluate lab results, perform pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) to adjust medication regimens.
  • Patient Education: Instruct patients and caregivers on safe use of infusion pumps, sterile techniques, and catheter maintenance for home care.
  • Care Coordination: Collaborate with physicians, home health nurses, and internal teams to support effective therapy management and reduce hospital readmissions.
  • Regulatory Compliance: Ensure adherence to State Board of Pharmacy regulations along with HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA standards.
  • Inventory & Shipping: Oversee pharmacy inventory including refrigerated medications and prepare shipments to patients’ homes.

Qualifications:

  • 1-3 years of experience in infusion specialty pharmacy or hospital IV room operations.
  • IVIg experience is advantageous.
  • Solid understanding of specialty therapy payer requirements, reimbursement, and contract obligations.
  • Active Arizona pharmacist license in good standing.
  • Familiarity with CareTend system preferred.

Schedule:

  • Monday through Friday, 8:30 a.m. to 5:00 p.m.
  • May include on-call rotation.

This contract opportunity is based in Chandler, AZ, with an immediate start and runs 8 to 12 weeks. Competitive hourly pay is offered, commensurate with experience.
