A dedicated clinical pharmacist role is available in Memphis, TN, focusing on home infusion therapy within a direct hire setting. This position is key to preparing, dispensing, and monitoring complex intravenous and subcutaneous medications, acting as the central coordinator within a multidisciplinary care team. The role offers a Monday to Friday schedule, 8 a.m. to 5 p.m., with participation in an on-call rotation averaging fewer than five after-hours calls per week.
Key Responsibilities:
- Sterile Compounding: Prepare specialty pharmaceuticals and IV solutions, ensuring compliance with all state and federal sterile compounding regulations.
- Clinical Monitoring: Evaluate lab values, perform pharmacokinetic dosing calculations (e.g., vancomycin), monitor Total Parenteral Nutrition (TPN), and make clinical intervention recommendations.
- Patient Onboarding & Education: Provide thorough education to patients and caregivers on infusion pump use, sterile techniques, and catheter care.
- Care Coordination: Work collaboratively with prescribing physicians, home health nurses, and internal teams including intake, referral services, reimbursement, clinical, and pharmacy operations to ensure cohesive therapy management.
- Regulatory Compliance: Adhere to standards set by State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable agencies.
- Inventory & Shipping: Oversee pharmacy inventory, including refrigerated items, and prepare medication shipments for safe and prompt home delivery.
Qualifications:
- Minimum of three years’ experience in infusion specialty pharmacy.
- Experience with IV immunoglobulin (IVIg) and hemophilia treatments strongly preferred.
- Proficient knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations.
- Current and good standing Tennessee pharmacist license.
- Familiarity with CareTend system.
