An infusion/clinical pharmacist position is available for direct hire in Memphis, TN. This role is centered on providing advanced pharmacy services within a home infusion setting, preparing and monitoring complex intravenous (IV) and subcutaneous medication therapies while working closely with a multidisciplinary healthcare team.
Schedule:
- Monday to Friday, 8:00 AM – 5:00 PM
- Participate in on-call rotation (typically fewer than five after-hours calls per week)
Key Responsibilities:
- Sterile Compounding: Accurately prepare specialty pharmaceuticals and IV solutions in compliance with state and federal regulations to ensure aseptic conditions.
- Clinical Monitoring: Evaluate lab values, perform pharmacokinetic dosing calculations for medications such as vancomycin, monitor Total Parenteral Nutrition (TPN), and adjust treatment plans accordingly.
- Patient Onboarding & Education: Provide thorough counseling and instruction to patients and caregivers on the use of infusion pumps, sterile techniques, and catheter maintenance.
- Care Coordination: Work collaboratively with prescribing physicians, home health nurses, and internal teams—including intake, referral services, reimbursement, and clinical operations—to ensure effective therapy management and reduce hospital readmissions.
- Regulatory Compliance: Adhere to all relevant regulations including State Board of Pharmacy standards, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and others.
- Inventory & Shipping: Oversee pharmacy inventory management, including temperature-sensitive products, and coordinate the packing and shipment of medications and supplies to patients’ homes.
Qualifications:
- Minimum of 3 years’ experience in infusion specialty pharmacy
- IV Immunoglobulin (IVIg) experience is highly preferred
- Experience in hemophilia therapy is highly preferred
- Strong understanding of specialty therapy payer requirements, reimbursement processes, and contract obligations
- Current and in good standing Tennessee pharmacist license
- Familiarity with CareTend pharmacy management system
