A home infusion clinical pharmacist is needed for an 8-12 week contract, providing critical expertise in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications. This role plays a key part as the central hub in a multidisciplinary care team, ensuring optimal therapy outcomes for patients receiving treatment at home.
Location: Glendale, AZ
Schedule: Monday to Friday, 8:30 AM – 5:00 PM; may include on-call rotation
Contract Duration: 8-12 weeks, starting ASAP
Key Responsibilities:
- Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment, adhering strictly to state and federal regulations
- Monitor clinical parameters by assessing lab values, calculating pharmacokinetic dosing (e.g., vancomycin), and managing Total Parenteral Nutrition (TPN) adjustments
- Provide thorough patient and caregiver education on infusion pump use, sterile techniques, and catheter maintenance
- Coordinate care with prescribing physicians, home health nurses, and internal teams to ensure smooth therapy management and minimize hospital readmissions
- Maintain compliance with regulatory bodies including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA standards
- Manage pharmacy inventory, including refrigerated medications, and oversee packing and shipping for timely home delivery
Desired Experience and Qualifications:
- 1-3 years of infusion specialty pharmacy experience or hospital pharmacy experience with IV room responsibility
- IVIg therapy knowledge is a plus
- Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations
- Current, in-good-standing pharmacist license in Arizona
- Experience with the CareTend system preferred
