Key Responsibilities:
- Prepare and dispense medications for home infusion therapy accurately and safely
- Conduct thorough reviews of medication orders and patient histories
- Monitor patient responses to therapy and provide clinical support as needed
- Collaborate with healthcare providers and patients to optimize treatment outcomes
- Maintain compliance with pharmacy laws, regulations, and standards
- Document all pharmacy-related activities in patient records
Qualifications & Desired Experience:
- Licensure as a Pharmacist with current authorization to practice
- IV Certification and demonstrated proficiency in home infusion pharmacy operations
- Previous experience in home infusion or related pharmacy practice
- Strong knowledge of medication management and patient counseling
- Excellent communication and organizational skills
- Ability to work independently and handle per diem assignments with flexibility
