Staff Pharmacist IV Certified - Houston, TX

A home infusion pharmacy with multiple nationwide locations seeks a Staff Pharmacist to join their Houston, TX site on a contract-to-hire basis. This role focuses on managing complex specialty and infusion therapies, ensuring patients receive high-quality care through expert medication management and coordination with healthcare providers.

Schedule:

• Monday to Friday, 8:30 AM to 5:00 PM

Key Responsibilities:

• Provide specialized clinical support, including managing specialty therapies and responding to drug information requests from patients and providers.
• Compound and dispense complex medications, primarily in infusion settings, ensuring accuracy and compliance.
• Collaborate closely with physicians, nurses, and insurers to coordinate medication delivery and administration.
• Educate patients on medication usage, potential side effects, and self-administration techniques, particularly for home infusion therapies.
• Ensure adherence to state and federal pharmacy regulations, maintain accurate patient records, and promote a safe and compliant practice environment.

Qualifications and Desired Experience:

• Licensed to practice pharmacy in Texas.
• ACPE Sterile Compounding certification is required.
• Minimum of 1-2 years of infusion pharmacy experience.
• Experience with CareTend pharmacy system is preferred.
• IV certification is mandatory.
