Staff Pharmacist IV Certified in Houston, TX

A home infusion pharmacy is seeking a Staff Pharmacist to join their Houston, TX location on a contract-to-hire basis. This role focuses on managing complex, specialty, and infusion therapies while coordinating patient care in an onsite setting.

Schedule:

• Monday to Friday, 8:30 a.m. to 5:00 p.m.

Key Responsibilities:

• Provide specialized clinical support 24/7, managing specialty therapies and delivering accurate drug information to patients and healthcare providers.
• Compound and dispense complex medications, especially in infusion settings, ensuring precision and safety.
• Coordinate with physicians, nurses, and insurers to facilitate timely medication delivery and administration.
• Educate patients on medication use, possible side effects, and home infusion self-administration techniques to promote adherence and safety.
• Maintain compliance with all state and federal pharmacy regulations, uphold accurate patient records, and ensure a safe and compliant work environment.

Qualifications and Desired Experience:

• Active Texas pharmacist license.
• ACPE/Sterile Compounding certification is required.
• Minimum of 1 to 2 years of experience in infusion pharmacy.
• Experience with the CareTend system is preferred.
• Strong attention to detail and patient-oriented approach.
