A clinical infusion pharmacist opportunity is available to support home infusion operations in the Syracuse, NY area. This direct hire position offers a Monday through Friday schedule, 8:00 a.m. to 4:30 p.m., with participation in on-call rotation. The ideal candidate will play a critical role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications to optimize patient outcomes in a home setting.
Key Responsibilities:
- Prepare specialty pharmaceuticals and IV solutions in sterile environments compliant with state and federal regulations.
- Monitor lab values, perform pharmacokinetic dosing calculations (e.g., vancomycin), and adjust Total Parenteral Nutrition (TPN) treatments as needed.
- Provide thorough patient and caregiver education on medication use, infusion pumps, sterile techniques, and catheter care.
- Collaborate with physicians, nurses, and internal teams to coordinate care and reduce hospital readmissions.
- Ensure compliance with regulations including HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and State Board of Pharmacy.
- Manage pharmacy inventory, including refrigerated medications, and oversee packaging for timely home delivery.
Qualifications:
- Minimum of 2 years’ experience in infusion specialty pharmacy.
- IV Immunoglobulin (IVIg) experience is a plus.
- Strong knowledge of specialty therapy payer requirements, reimbursement, and contract terms.
- Current, valid New York pharmacist license in good standing.
- Experience with the CareTend system preferred.
