A clinical pharmacist with a specialty in home infusion is sought for a direct hire opportunity based in Memphis, TN. This role offers a Monday through Friday schedule from 8 a.m. to 5 p.m., with participation in an on-call rotation that typically requires fewer than five after-hours calls weekly.

Key Responsibilities:

  • Prepare sterile compounding of specialty pharmaceuticals and intravenous solutions, adhering strictly to state and federal regulations.
  • Monitor and adjust medication therapies using clinical data, including pharmacokinetic dosing and Total Parenteral Nutrition (TPN) management.
  • Provide thorough patient and caregiver education on infusion pump use, sterile techniques, and catheter care at home.
  • Collaborate with physicians, nurses, and multidisciplinary teams to coordinate care and promote optimal patient outcomes, including reducing hospital readmissions.
  • Ensure compliance with all relevant regulatory standards such as State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, among others.
  • Manage pharmacy inventory, including refrigerated medications, and coordinate packing and shipping for timely delivery to patients' homes.

Qualifications:

  • Minimum of three years’ experience in infusion specialty pharmacy.
  • IVIg and hemophilia experience are highly preferred.
  • Strong understanding of specialty therapy payer requirements, reimbursement procedures, and contractual obligations.
  • Current Tennessee pharmacist license in good standing.
  • Familiarity with CareTend system is a plus.
