Home Infusion Staff Pharmacist - Modesto, California

A growing home infusion pharmacy is seeking a skilled Staff Pharmacist with IV certification to support patient care and medication management. This contract opportunity offers an engaging role providing pharmacy services in a home infusion setting, ensuring the safe preparation and dispensing of intravenous medications.

Key Responsibilities:

• Accurately verify and dispense prescriptions for home infusion patients
• Prepare and review IV admixtures ensuring compliance with safety protocols
• Counsel patients and caregivers on medication use and administration
• Collaborate with healthcare providers to optimize pharmacotherapy outcomes
• Maintain documentation in accordance with regulatory and company standards

Desired Experience and Qualifications:

• Licensed Pharmacist with current California license
• IV Certification is required
• Previous experience working in home infusion pharmacy preferred
• Strong knowledge of compounding sterile preparations and USP <797> guidelines
• Excellent communication and patient care skills
