Home Staff Pharmacist | New York, New York

A healthcare facility in New York, NY is seeking two experienced pharmacists for a 13-week contract with a potential for contract-to-hire based on performance. This opportunity focuses on home infusion pharmacy services, requiring IV certification and prior experience in similar roles. The position operates Monday through Friday from 8:30 AM to 5:00 PM.

Key Responsibilities:

• Accept and dispense prescription medications accurately and lawfully.
• Monitor patient medication adherence and over-usage patterns.
• Ensure medications prescribed are suitable and comply with legal regulations.
• Maintain confidential patient records and pharmacy documentation meticulously.
• Provide patients with detailed medication counseling, including usage, side effects, and interactions.
• Deliver additional patient services such as smoking cessation support, blood pressure monitoring, and cholesterol management.
• Educate patients on proper use and handling of medical devices.
• Compound medications and prepare sterile solutions following established protocols.
• Supervise pharmacy staff including technicians and clerks.
• Manage inventory of pharmaceutical and medical supplies.
• Collaborate effectively with physicians, nurses, and other healthcare professionals to address drug-related issues.

Qualifications and Experience:

• Doctor of Pharmacy (PharmD) degree.
• Active New York State pharmacy license.
• Previous IV certification and solid experience with IV pharmacy procedures.
• Strong knowledge of home infusion pharmacy practices.
• Proven ability to maintain patient confidentiality and comply with healthcare regulations.
• Excellent communication and supervisory skills.
