This role offers the opportunity to support patients by preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications in a home infusion setting. Acting as a clinical expert, you will oversee sterile compounding, adjust medication doses based on laboratory results, and provide essential education to patients and caregivers. This position is contracted for 8-12 weeks with an immediate start and requires collaboration with a multidisciplinary team to ensure seamless therapy management.

Location:

  • Pearland, TX (Houston metro area)

Schedule:

  • Monday to Friday, 8:30 a.m. – 5:00 p.m.
  • May require participation in on-call rotation

Key Responsibilities:

  • Prepare specialty pharmaceuticals and IV solutions in compliance with all state and federal regulations within a sterile environment
  • Monitor lab values, calculate pharmacokinetic dosing (e.g., vancomycin), and adjust Total Parenteral Nutrition (TPN) treatments accordingly
  • Provide comprehensive counseling and education to patients and caregivers on medication use, infusion pump operation, sterile techniques, and catheter maintenance
  • Collaborate with physicians, home health nurses, and interdisciplinary teams to optimize patient therapy and reduce hospital readmissions
  • Maintain compliance with regulatory standards including State Board of Pharmacy, HIPAA, FDA, DEA, ACHC, URAC, and OSHA guidelines
  • Manage pharmacy inventory, including refrigerated products, and ensure safe, timely shipment of medications to patient homes

Desired Experience and Qualifications:

  • 1-3 years of infusion specialty pharmacy or hospital pharmacy experience, particularly within an IV room
  • IVIg experience is a plus
  • Familiarity with specialty therapy payer requirements, reimbursement processes, and contracting obligations
  • Current and active Texas pharmacist license in good standing
  • Experience with CareTend or similar pharmacy management systems
