An opportunity is available for a Staff Clinical/Infusion Pharmacist to support home infusion services on an 8-12 week contract, starting immediately. This role focuses on preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications, ensuring high-quality patient care in a home setting. The position is based in Phoenix, AZ, with a Monday through Friday schedule from 8:30 a.m. to 5:00 p.m., and includes potential on-call rotation.

Responsibilities include:

  • Sterile compounding of specialty pharmaceuticals and IV solutions in compliance with state and federal regulations
  • Clinical monitoring and adjustment of pharmacokinetic dosing, including Total Parenteral Nutrition (TPN), with thorough patient counseling about specialty medications and treatment plans
  • Educating patients and caregivers on infusion pump use, sterile techniques, and catheter maintenance at home
  • Coordinating care with physicians, home health nurses, and multidisciplinary teams to ensure effective therapy management and reduce hospital readmissions
  • Ensuring regulatory compliance with State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other standards
  • Managing pharmacy inventory, including refrigerated products, and preparing medications for timely and safe delivery

Qualifications or Desired Experience:

  • 1-3 years of experience in infusion specialty pharmacy or in a hospital pharmacy IV room role
  • IVIg experience is a plus
  • Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations
  • Current and valid Arizona pharmacist license in good standing
  • Experience using CareTend system preferred
