A clinical infusion pharmacist role is available for a contract assignment lasting 8–12 weeks, starting ASAP, in Sugar Land, TX. This position supports home infusion therapy by preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications, serving as a key member of a multidisciplinary care team.

Schedule:

  • Monday to Friday, 8:30 a.m. to 5:00 p.m.
  • Possible participation in on-call rotation

Core Responsibilities:

  • Sterile compounding of specialty pharmaceuticals and IV solutions in compliance with federal and state regulations
  • Clinical monitoring, including assessment of lab values, pharmacokinetic dosing calculations (e.g., vancomycin), and Total Parenteral Nutrition (TPN) management
  • Providing thorough patient and caregiver education on medication use, infusion pumps, sterile technique, and catheter care
  • Coordinating care with prescribing physicians, home health nurses, and internal teams to optimize therapy and reduce hospital readmissions
  • Maintaining regulatory compliance with agencies such as the State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA
  • Managing pharmacy inventory, including refrigerated products, and preparing shipments for safe and timely patient delivery

Qualifications:

  • Active Texas pharmacist license in good standing
  • 1–3 years of experience in infusion specialty pharmacy or hospital pharmacy with a focus on IV room responsibilities
  • Familiarity with specialty therapy payer requirements, reimbursement, and contract obligations
  • IVIg experience is a plus
  • Experience with CareTend system preferred
