A home infusion clinical pharmacist role is available for a direct hire opportunity with an immediate start. The position involves preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications, acting as a key member of a multidisciplinary team supporting patient care in home infusion settings.
Schedule & Hours:
- Monday to Friday, 8:00 AM to 4:30 PM
- Participate in on-call rotation
Key Responsibilities:
- Sterile Compounding: Prepare specialty pharmaceuticals and IV solutions in sterile, ventilated environments adhering to all regulatory requirements.
- Clinical Monitoring: Review lab values, calculate pharmacokinetic dosing (e.g., vancomycin), monitor Total Parenteral Nutrition (TPN), and adjust treatments accordingly. Provide comprehensive patient counseling on specialty medications and treatment plans.
- Patient Onboarding & Education: Instruct patients and caregivers on infusion pump use, sterile technique, and catheter maintenance at home.
- Care Coordination: Work collaboratively with physicians, home health nurses, and internal teams to ensure effective therapy management and reduce hospital readmissions.
- Regulatory Compliance: Maintain adherence to State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards.
- Inventory & Shipping: Oversee pharmacy inventory, including refrigerated products, and manage safe packaging and timely delivery of medications to patients’ homes.
Qualifications & Experience:
- Minimum of 2 years’ experience in infusion specialty pharmacy
- IVIg experience is a plus
- Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations
- Current and valid NY pharmacist license in good standing
- Experience with CareTend system preferred
