Contract Clinical Remote MTM Pharmacist -Monday to Friday Day Schedule PBM Experience

Soliant Health is teaming up with a Pharmacy in search of a remote MTM Pharmacist for a contract spanning April to October, with potential for extension. This role requires licensure in two states from the list of LA, NJ, GA, KY, with UT and TX licenses viewed as beneficial. Position Details:
-Position: MTM / Medication Adherence Pharmacist
- Contract Type: Temporary (5-6 months), with potential for extension
- Start Date: Mid-April
- End Date: Through October 2024
- Hours: Monday - Friday, 9:00 am - 5:00 pm EST (flexibility required as hours may expand)

Responsibilities:
- Conduct remote medication adherence/management calls, providing direct patient outreach and support.
- Prevent, detect, and resolve medication-related problems, offering evidence-based, patient-centered medication and therapy recommendations.
- Perform thorough medication reconciliations for post-discharge patients.
- Assist in managing high-risk, high-cost patients to ensure optimal care.
- Conduct telephonic comprehensive medication reviews (CMRs) for qualified patients.
- Identify and address patient-specific barriers to medication adherence and disease state management.
- Provide clinical decision support, education, and consultations to providers as needed.
- Enhance coordination of care between care managers, providers, and pharmacies to optimize medication management.
- Collaborate with providers, clinical staff, and administrators to improve medication management workflows and patient experiences.
- Adjust and recommend alternative pharmacotherapy based on formulary changes and evidence-based guidelines.

Experience Requirements:
- Strong background in MTM, preferably with experience in health plan or pharmacy benefit management (PBM) settings.
- Retail pharmacists with MTM experience will be considered, provided they have performed similar duties.
 
Licensure Requirements:
- Ideal candidate must possess licensure in at least two of the following states: LA, NJ, GA, KY (bonus states: UT, TX).
