A closed-door long term care (LTC) pharmacy in a Northeast suburb of Cincinnati is seeking a dedicated clinical pharmacist for a direct hire opportunity in Mason, OH. This role emphasizes clinical expertise and operational support within a patient-focused environment.
Key Responsibilities:
- Review and verify all new patient medication orders, ensuring accuracy and proper documentation.
- Collaborate closely with physicians and healthcare professionals to plan and execute safe and effective drug therapies for patients at home.
- Conduct comprehensive assessments of patient-specific drug and disease information to identify potential medication-related issues.
- Establish therapeutic goals and monitoring parameters alongside clinical staff.
- Monitor and interpret clinical and laboratory data to evaluate drug therapy effectiveness.
- Communicate therapeutic findings and recommendations clearly to physicians, nursing staff, and other care team members.
- Provide expert consultation to patients, families, and healthcare providers regarding medication use.
- Maintain detailed clinical records and participate in ongoing education to stay current with pharmacotherapy developments.
- Manage formulary substitutions and ensure drug therapy aligns with diagnoses for optimal outcomes.
- Oversee medication processing, supply inventory, and equipment dispensing according to regulatory standards.
- Participate in weekend, holiday, and on-call rotations as well as staff training and departmental meetings.
- Ensure compliance with all federal, state, and local pharmacy laws governing the profession.
Qualifications:
- 1-3 years of experience in long term care pharmacy.
- Familiarity with pharmacy management systems, with preference given to experience using Frameworks.
- Strong clinical assessment and communication skills.
- Knowledge of regulatory requirements affecting pharmacy practice.
