Staff Pharmacist in Middletown, OH

This direct hire opportunity is for a Staff Pharmacist role supporting a closed-door long-term care (LTC) pharmacy located in Mason, OH, a Northeast Cincinnati suburb. The position focuses primarily on clinical pharmacy services with additional operational and regulatory responsibilities, providing comprehensive pharmaceutical care for patients receiving home care.

Key Responsibilities:

• Review and verify new patient medication orders according to organizational policies.
• Collaborate with physicians and healthcare professionals to plan and implement safe, effective drug therapies for home care patients.
• Conduct initial and ongoing clinical assessments to identify and resolve drug-related problems.
• Assist in setting therapeutic objectives and monitor safety and efficacy by evaluating lab results and clinical data.
• Provide pharmacological consultations to patients, families, physicians, and care teams.
• Document all clinical activities and communications thoroughly in patient records.
• Maintain up-to-date clinical knowledge through literature review and professional development.
• Process and check medications, supplies, and equipment for dispensing.
• Maintain pharmacy records in compliance with legal and regulatory requirements.
• Participate in orientation, training, meetings, and weekend/holiday/on-call rotations.
• Dispense pharmaceuticals adhering to all federal, state, and local laws and standards.

Qualifications:

• 1 to 3 years’ experience in long-term care pharmacy.
• Familiarity with Frameworks pharmacy system is highly preferred but not mandatory.
• Strong clinical assessment and communication skills.
• Knowledge of regulatory and operational pharmacy practices.
