Key Responsibilities
- Deliver comprehensive ambulatory clinical pharmacy services as a credentialed member of the healthcare team through Collaborative Practice Agreements (CPAs)
- Conduct patient visits involving medication reconciliation, lab monitoring, counseling, and disease state management
- Collaborate with prescribers and multidisciplinary teams to develop and document safe, evidence-based therapeutic plans aligned with Comprehensive Medication Management (CMM) practices
- Manage specialty medication care coordination including initiation, monitoring, and continuity of therapy
- Facilitate patient access through prior authorizations, financial assistance facilitation, and benefit investigations
- Provide drug information and education to patients, families, and healthcare providers
- Serve as a clinical resource and precept pharmacy students, interns, or residents
- Participate in quality improvement initiatives, research, committee work, and policy development
Qualifications / Desired Experience
- Active Delaware Pharmacist License in good standing required
- Board of Pharmacy Specialties (BPS) Certification in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics – mandatory
- At least one year clinical pharmacy experience, including residency
- Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD preferred)
- Experience with Collaborative Practice Agreements and specialty medication management preferred
- Familiarity with Comprehensive Medication Management (CMM) and chronic disease management advantageous
- Preceptor experience is a plus
