Key Responsibilities

• Deliver comprehensive ambulatory clinical pharmacy services as a credentialed member of the healthcare team through Collaborative Practice Agreements (CPAs)
• Conduct patient visits involving medication reconciliation, lab monitoring, counseling, and disease state management
• Collaborate with prescribers and multidisciplinary teams to develop and document safe, evidence-based therapeutic plans aligned with Comprehensive Medication Management (CMM) practices
• Manage specialty medication care coordination including initiation, monitoring, and continuity of therapy
• Facilitate patient access through prior authorizations, financial assistance facilitation, and benefit investigations
• Provide drug information and education to patients, families, and healthcare providers
• Serve as a clinical resource and precept pharmacy students, interns, or residents
• Participate in quality improvement initiatives, research, committee work, and policy development

Qualifications / Desired Experience

• Active Delaware Pharmacist License in good standing required
• Board of Pharmacy Specialties (BPS) Certification in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics – mandatory
• At least one year clinical pharmacy experience, including residency
• Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD preferred)
• Experience with Collaborative Practice Agreements and specialty medication management preferred
• Familiarity with Comprehensive Medication Management (CMM) and chronic disease management advantageous
• Preceptor experience is a plus
