A clinical pharmacist role is available specializing in home infusion therapy, focusing on preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications. This position serves as a crucial member of a multidisciplinary team, overseeing sterile compounding, dosing adjustments, and patient education to ensure optimal treatment outcomes.

Location: Phoenix, AZ
Job Type: Contract
Duration: Approximately 8-12 weeks, starting ASAP
Schedule:

  • Monday to Friday, 8:30 a.m. to 5:00 p.m.
  • On-call rotation may be required

Key Responsibilities:

  • Prepare specialty pharmaceuticals and IV solutions in a sterile environment compliant with all relevant regulations.
  • Monitor labs, calculate pharmacokinetic dosing (e.g., vancomycin), and adjust Total Parenteral Nutrition (TPN) therapies.
  • Provide comprehensive counseling and education to patients and caregivers on medication use, infusion pumps, sterile techniques, and catheter maintenance.
  • Coordinate care with physicians, home health nurses, and pharmacy teams to support seamless therapy management and minimize hospital readmissions.
  • Ensure compliance with State Board of Pharmacy regulations and federal standards including HIPAA, FDA, DEA, Medicare, and OSHA.
  • Manage pharmacy inventory, including refrigerated products, and prepare medications for timely and safe delivery to patients’ homes.

Qualifications and Experience:

  • 1 to 3 years experience in infusion specialty pharmacy or hospital pharmacy with responsibilities in the IV room.
  • Experience with IVIg preferred.
  • Understanding of specialty therapy payer requirements, reimbursement, and contract obligations.
  • Current and good standing pharmacist licenses in Arizona and Texas.
  • Familiarity with CareTend system is beneficial.
