This position offers an opportunity to serve as an Ambulatory Clinical Pharmacy Specialist within a well-established healthcare network. The role emphasizes direct clinical involvement, providing comprehensive medication management to patients in an outpatient clinic setting. This role supports chronic disease management and specialty medication care coordination while collaborating closely with prescribers and healthcare teams.

Schedule:
- Monday through Friday
- 8:00 AM to 4:30 PM or 8:30 AM to 5:00 PM
- No weekends or holidays
Responsibilities:
- Deliver ambulatory clinical pharmacy services under Collaborative Practice Agreements, including assessment, monitoring, and management of therapeutic plans.
- Conduct patient visits to perform medication reconciliation, laboratory monitoring, counseling, and disease state management.
- Collaborate with providers and multidisciplinary teams to implement evidence-based medication interventions focusing on quality, safety, and cost reduction.
- Manage referrals with closed-loop communication to referring clinicians.
- Support patient access through prior authorization coordination, financial assistance programs, and benefit investigations.
- Oversee specialty medication therapy, ensuring safe initiation, monitoring, and continuity.
- Provide drug information and education to patients, families, and healthcare team members.
- Precept pharmacy students, interns, and residents as assigned.
- Participate in process improvement, research, publications, committee workgroups, and policy development.
- Document interventions and report outcomes consistent with ambulatory pharmacy practice standards.
Qualifications and Desired Experience:
- Active Delaware Pharmacist License in good standing (or ability to obtain quickly).
- Board of Pharmacy Specialties (BPS) certification in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics (mandatory).
- Minimum one year clinical pharmacy experience, including residency experience.
- Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD strongly preferred).
- Experience practicing under Collaborative Practice Agreements preferred.
- Specialty medication management and prior authorization experience preferred.
- Completion of PGY1 or PGY2 ambulatory care residency is a strong plus.
- Experience with Comprehensive Medication Management (CMM), chronic disease management, or population health pharmacy highly valued.
- Preceptor experience is a plus.
