A direct hire opportunity is available for a Staff Pharmacist to join a closed-door long-term care pharmacy located in Mason, OH, a northeast suburb of Cincinnati. This role involves providing clinical, operational, and regulatory pharmacy services to support safe and effective drug therapy for patients in the home setting.
Key Responsibilities:
- Review, verify, and document all new patient orders while ensuring adherence to organization policies.
- Collaborate with physicians and healthcare professionals to develop and implement patient care plans.
- Conduct initial and ongoing assessments to identify drug-related problems and establish therapeutic objectives.
- Monitor clinical and laboratory data to evaluate drug therapy effectiveness and safety.
- Provide expert consultation and respond to inquiries from patients, families, and healthcare staff.
- Maintain accurate documentation of all clinical activities and communications.
- Stay current with pharmacotherapy literature and participate in ongoing education.
- Process and check medications, supplies, and equipment prior to dispensing.
- Maintain pharmacy records in compliance with relevant laws and regulations.
- Participate in weekend, holiday, and on-call rotations as well as staff training and departmental meetings.
- Dispense pharmaceutical supplies in line with federal, state, and local regulatory standards.
Qualifications and Experience:
- Minimum 1-3 years’ experience in a long-term care pharmacy setting.
- Familiarity with the Frameworks system is highly preferred but not mandatory.
- Strong clinical assessment and communication skills.
- Ability to work independently and as part of a multidisciplinary team.
- Knowledge of pharmacy regulations and standards of practice.
