A Long-Term Care (LTC) closed-door pharmacy in Mason, OH, seeks an experienced pharmacist for a direct hire opportunity focused on clinical and operational excellence. This role offers the chance to contribute meaningfully to patient care within a specialized healthcare setting serving the Northeast Cincinnati area.

Key Responsibilities:

• Review, verify, and document new patient medication orders accurately.
• Collaborate with physicians and healthcare teams to develop and implement safe, effective drug therapies.
• Conduct thorough patient assessments to identify drug-related problems and optimize therapeutic outcomes.
• Monitor clinical and laboratory data to evaluate drug therapy effectiveness.
• Provide consultation and answer inquiries related to medication use from patients, families, and healthcare professionals.
• Maintain detailed clinical documentation, ensuring regulatory compliance.
• Perform medication dispensing and quality checks prior to distribution.
• Participate in weekend, holiday, and on-call rotations.
• Assist in staff orientation and training; engage in department meetings.
• Ensure all pharmaceutical activities comply with federal, state, and local laws.

Desired Experience and Qualifications:

• 1 to 3 years of pharmacy experience in long-term care settings.
• Familiarity with Frameworks pharmacy system preferred.
