Schedule/Hours:
- 40 hours per week (may be 4x10’s or 5x8’s, based on operational needs)
- Day shift, Monday through Friday
Qualifications:
- Active RN license in good standing
- Minimum of 2 years of experience in case management
- At least 6 months of discharge planning case management experience within an acute care hospital
- Preferred experience in a large organization or teaching hospital with 500+ beds
- Strong communication, critical thinking, and organizational skills
- Ability to work collaboratively with multidisciplinary teams
Key Responsibilities:
- Assess, coordinate, and monitor all aspects of patient care to ensure optimal outcomes and resource utilization
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
- Facilitate discharge planning processes and ensure smooth transitions of care
- Advocate for patients and families by providing education and support throughout the care continuum
- Collaborate with social workers, physicians, and other healthcare professionals to align care goals
- Navigate complex clinical and psychosocial situations to support patients’ health and recovery

$2,299 - $2,432/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
