This contract role supports a Level 1 trauma hospital, working across all inpatient units and collaborating within a robust interdisciplinary team—including a dedicated Social Work team and specialized consults with Pharmacy and Respiratory Therapy.
Schedule:
- Monday to Friday, 8:00 a.m. – 4:30 p.m.
- No weekend shifts
- Holiday coverage rotates as needed
Key Qualifications:
- Active RN license with a minimum of 2 years’ recent acute inpatient case management experience, including at least 1 year in the last 3 years in a Level 1/Academic Medical Center emergency department (500+ beds required)
- Demonstrated acute care discharge planning within the past 2 years, covering a wide range of medical diagnoses
- Prior travel experience in a Level 1 trauma facility within the last 3 years
- Proficient in EPIC charting and strong computer skills
- Experience exclusively in telephonic case management, utilization review, insurance, rehabilitation, LTC, or hospice will not be considered
- Compliance with facility requirements including influenza vaccination (medical exemption only with provider documentation) and fingerprinting
Role Responsibilities:
- Manage a daily caseload of 15–20 patients/consults, collaborating with a large Social Work team for comprehensive care
- Lead acute care case management and multidisciplinary discharge planning for patients with complex and varying needs
- Coordinate placement in partner facilities while ensuring eligibility criteria, working with limited resources for uninsured or underinsured populations
- Contribute to care conferences, advocating for patient-centered solutions in a fast-paced, academic environment
- Participate in departmental orientation; independent function expected following the initial training period
