Schedule/Hours:
- 5x8 hour shifts: 40 hours per week
- 36/48 rotating schedule (7a-7p every Wednesday and Thursday, and every other Friday, Saturday, Sunday)
- No regular weekend requirements; holidays rotated as needed
Qualifications:
- Active LMSW or LCSW and MS state license
- Minimum 2 years of recent acute inpatient medical case management experience (with at least 1 year in a large, Level 1 hospital/medical center)
- Prior travel in a Level 1 facility within the last 3 years
- Strong computer proficiency; EPIC charting required
- Acute care and ED discharge planning expertise
- BLS and Compact State License
- Inpatient acute case management and discharge planning experience is essential
Responsibilities:
- Manage a caseload of 15-20 patients daily
- Collaborate with interdisciplinary teams, RT, and Pharmacy as needed for discharge planning
- Provide seamless coordination for complex placements and support uninsured/underinsured patients
- Utilize strong clinical documentation, resource referrals, and patient advocacy skills
